                ...to release a copy of my clearances which are currently in date and on file to the Secretary at the Anthracite Baseball Association (ABA) for coaching or umpiring in the youth baseball program. They can be scanned and sent to anthracitebaseball@gmail.com

 

 Those clearances are:  Act 34 (Pennsylvania Access to Criminal History) and Act 151 Pennsylvania Child Abuse History.

 

If the district has Act 114 FBI Fingerprints on file, I also give permission for those to be sent as well. 

 

I further understand if there are no Act 114 FBI Fingerprints on file, I must submit the affidavit that can be found on the ABA website.
